This Notice describes how Medical Information about You (the patient), described as PHI or Private Health Information, may be used and disclosed and how you can get access to this information. Please review it carefully.
Uses and Disclosures: This entity may disclose your personal health information for the purpose of health care payments and remittance advice. We may, with your consent, send personal health information to disability insurance companies. We may disclose information for coordination of benefits, health care claim status reports, health care enrollments and disenrollments, health plan eligibility information transmissions and health care premiums. It may be requested that we release your personal health information for referral or consultation, second opinion or for HMO referrals or authorizations necessary for second opinions also for case presentation for tumor conference/ tumor board, or authorizations for use or disclosure of information for research purposes through treatment. We may be required to provide your protected health information if requested to do so legally, such as a summons.
Special Uses: Andrews & Patel Associates, P.C. may use protected information for special purposes such as appointment reminder notices or rescheduling notices. If you do not want this please make a special request.
Statement of Individual Rights: You have the following rights: The right to request restrictions on certain uses and disclosures of protected health information. The right to receive confidential communications of protected health information. Right to amend protected health information, right to receive an accounting of disclosures of protected health information on a yearly basis at your request, right of an individual, including an individual who has agreed to receive the notice electronically in accordance to paragraph c3 of the section, to obtain a paper copy of the notice from the covered entity upon request.
Covered Entity’s Duties: Andrews & Patel Associates, P.C. is required by law to maintain the privacy of protected health information and must provide patients with a notice of its legal duties and the privacy practices it is required to follow about protected health information. The covered entity must abide by the terms of the notice currently in effect. Before Andrews & Patel Associates, P.C. can change a privacy practice it must issue a statement to patients that it reserves the right to change the terms of its notice and put a new privacy practice in effect for all the protected health information it maintains. The revised notice will be posted in the office for 90 days prior to a change in privacy policy practice.
Complaints: Individuals may complain to the practice of Andrews & Patel Associates and to the Secretary of the Department of Health and Human Services if they believe their privacy rights have been violated. The complaint must be made in writing to the HIPAA Compliance Officer- Sharlene Bence- West Shore Office Manager 3912 Trindle Road Camp Hill, PA 17011. The practice of Andrews & Patel Associates, P.C. will in no way retaliate against the individual for filing a complaint.
